Essence
of the in vitro fertilization (IVF) procedure is to get mature
oocytes from woman ovaries and to fertilize these oocytes with
spermatozoa of husband (or donor's sperm if both spouses agree) and
to rise created embryos in the incubator during 48-72 hours period
with following embryo transfer to the patient's uterus.
Starting
from 2nd day of the menstrual cycle patient receives stimulating
tablets and injections. It helps to put into ovaries and to rise as
a rule not one but few oocytes.
In
ultrasonic research of ovaries and an uterus conducted approximately
from the day 8th to the day 14th, a doctor looks after the growth of
the follicles in ovaries and oocytes maturing in it. At some point
of the cycle (from 10th to 16th) injection is prescribed (usually at
night 23.00), that determines the moment of the final maturing and
punction of follicles.
In 36
hours after that injection all oocytes are taken from the patient
(Fig.1) (these oocytes are sucked with thinnest needle) and at the
same day man gives his sperm for IVF completion.
Before
fertilization the sperm being handled carefully to get as much
active spermatozoa as possible, to clean them of semen plasma,
leukocytes and other sperm elements which can affect the
fertilization.
Received
after the processing, spermatozoa will be added to the wife's
oocytes. Oocytes staying in the nutrient medium in special 4-well
multi dishes. Then, these dishes with oocytes and spermatozoas (with
detailed data about husband and wife) will be placed to the
incubator. Incubator electronic devices control strictly the
temperature, humidity, gas environment level, which is necessary for
normal fertilization and embryo growth.
In 18
hours embryologist picks dishes from the incubator to define
fertilization availability. If fertilization completed successfully
(fig.2) oocytes will be putted into special nutrient medium where
oocytes cleavage take place, means become embryos. This process
takes 16-18 hours too.
Embryo
transfer to the patient's uterus will be done (Fig.3), if the
embryologist sees normally growing embryos. Embryos should become
acclimatize in the uterus. In a case of success in 3 weeks after
embryo transfer ultrasonic research take place to check the number
of settled down embryos.
